The Direct Loan Program has a variety of forms available for use by Direct Loan borrowers. Available forms include: an optional form to select or change your repayment plan, required forms (marked with an *) for anyone repaying under the Income Contingent Repayment (ICR) plan, and optional forms to request a Loan Discharge or Cancellation.

Alternative Documentation of Income Form
This form is for reporting your income information to the Direct Loan Program under the Income Contingent Repayment (ICR) plan. We use income information to calculate your monthly payments each year. You need to complete and send us this form whenever we request it.
Child Care Provider Loan Forgiveness Program Form
The Child Care Provider Loan Forgiveness Program is a demonstration program that is intended to bring highly trained individuals into the early child care profession and to keep more highly trained child care providers in the early child care field for longer periods. Under this program, individuals who work full-time in certain child care facilities that serve low-income families and meet other qualifications may be eligible to have up to 100 percent of their Direct Loan and/or FFEL Program loans forgiven.
Closed School Discharge Form
If you cannot complete your education because your school closed and if you are not in the process of completing that program of study or comparable program at another school by transferring credit or hours earned at the closed school to another school, and you meet other criteria, you may be eligible for a Closed School Discharge. You would not need to repay any loan(s) we discharge.
Consent To Disclosure of Tax Information Form
This form is for giving your consent to the Direct Loan Program under the Income Contingent Repayment (ICR) plan to obtain your adjusted gross income (A.G.I.) from the Internal Revenue Service (IRS). We use income information to calculate your monthly payments each year. You need to complete the consent form and send it to us whenever we request it.
Electronic Debit Account (EDA) Form
Electronic Debiting is a service that allows your bank to automatically deduct your monthly payment(s) from your checking or savings account. You can download the EDA application and complete it on your own using the detailed instructions contained on the form.
False Certification of Ability to Benefit Discharge Form
If you did not have a GED or high School Diploma at the time you enrolled at the school, or you were not properly given an entrance examination to test your ability to benefit from the program of study, you may be eligible for a loan discharge. You would not need to repay any loan(s) we discharge.
False Certification Disqualifying Status Discharge Form
If you do not meet the legal requirements for employment in your state of residence after completing your education and meet other criteria, you may be eligible for a loan discharge. You would not need to repay any loan(s) we discharge.
False Certification Identity Theft Form
If you have been a victim of identity theft and are able to provide supporting documentation you may be eligible for a loan discharge. You would not need to repay any discharged loan(s)
Repayment Plan Selection Form
This form is used to select or change your repayment plan.
Teacher Loan Forgiveness Form
Under this program, individuals who teach full-time for five years in certain low-income schools and meet other requirements may be eligible for forgiveness of up to a combined total of $17,500 in principal and interest of their loan(s).
Total And Permanent Disability Discharge Request Form
This form is for requesting a discharge of remaining Direct Loan amounts after disability. You may be eligible (1) if a physician (doctor of medicine or osteopathy) certified that you are totally and permanently disabled and you meet other requirements during a three-year conditional discharge period; or (2) if you are a veteran and the U.S. Department of Veterans Affairs (VA) has determined that you are 100% disabled due to one or more service-connected disability conditions or that you are individually unemployable.
Unauthorized Signature/Unauthorized Payment Discharge Form
If it is not your signature on a Direct Loan application, promissory note, loan disbursement check, authorization of electronic funds transfer, and/or master check authorization and you meet other criteria, you may be eligible for a loan discharge. You would not need to repay any loan(s) we discharge.
Unpaid Refund Discharge Form
If a refund was due to you and it was never paid, it could cancel your obligation (and any endorser's obligation) to repay the portion of your student loan that should have been refunded. If you meet other criteria, you may be eligible for a loan discharge. You would not need to repay any portion of loan(s) we discharge.
