Application and Promissory Note Form
This form serves to collect application information for a new Direct Consolidation Loan. In addition, it also serves as the legal instrument by which you agree to abide by the terms of the Direct Consolidation Loan Program.
Cover Letter-Consolidation Package Form
This letter gives you a brief introduction about what you need to know before submitting your Consolidation application and during the application process.

This form provides instructions on how to complete your application and promissory note.
Mailing Instructions Form
This document shows the address where you should mail your application and promissory note and other forms related to your Consolidation application.
Additional Loan Listing Sheet Form
This form is used if all of your loans do not fit into the space provided on your application and promissory note. This form is to be returned with your Application and Promissory Note.
Repayment Plan Selection (RPS) Form Form
This form is used to select your repayment plan when you are applying for a Direct Consolidation Loan.
ICR Consent to Disclosure of Tax Information Form
If you select or are required to repay your new Direct Consolidation Loan under the Income Contingent Repayment (ICR) plan, submit this form giving your consent to the Direct Loan Program under the plan to obtain your adjusted gross income (A.G.I.) from the Internal Revenue Service (IRS). We use income information to calculate your monthly payments for your Consolidation Loan.
Request to Add Additional Loans Form
This form is for adding additional eligible loan(s) to an existing Direct Consolidation Loan. You may add additional loans up to 180 days after the date on which your Direct Consolidation Loan was made.
